A Qualitative Study on Psychosocial Struggles and Resilience among Transgender Individuals in Pakistan

Abstract

Transgender in Pakistan faces systemic discrimination, family rejection, and exclusion from social institutions. These psychosocial struggles are compounded by religious stigma and a lack of legal protection. This study investigates the psychosocial challenges faced by transgender men in Pakistan and explores their resilience strategies. Six participants aged 22–30 were interviewed using semi-structured questions and administered the Thematic Apperception Test (TAT). Thematic analysis and Interpretative Phenomenological Analysis were applied. Six themes emerged: identity confusion, mental health challenges, family neglect, religious stigma, spiritual coping, and resilience. Participant narratives highlighted depression, anxiety, and suicidal ideation, but also demonstrated self-acceptance, spirituality, and community solidarity. TAT excerpts revealed unconscious struggles of isolation and desire for recognition. Transgender men experience profound psychosocial distress, yet develop resilience through coping strategies. The findings call for inclusive policies, family awareness programs, and community-based interventions.
