Cardio–Renal–Metabolic Syndrome: From Pathophysiological Mechanisms to Precision Medicine and Emerging Therapeutic Strategies: A Comprehensive Narrative Review
The concept of cardio-renal-metabolic (CKM) syndrome addresses a multisystem disorder linking cardiovascular diseases, chronic kidney disease, and metabolic disorders such as obesity and type 2 diabetes. These conditions are highly interconnected and influence one another through a complex network of chronic inflammation, oxidative stress, insulin resistance, and neurohormonal and vascular imbalances. This comprehensive narrative review explores the underlying pathophysiological mechanisms, identifying visceral obesity and adipose tissue dysfunction as critical starting points for a cascade of inflammatory reactions that damage blood vessels, kidneys, and the heart. It elucidates the crucial roles of the renin-angiotensin-aldosterone system (RAAS), endothelial dysfunction, and fibrosis in accelerating disease progression. Furthermore, the review discusses the clinical importance of early diagnosis, personalized risk stratification, and the application of precision medicine tailored to individual genetic, biological, and clinical profiles. From a therapeutic standpoint, we highlight the synergistic effectiveness of modern pharmacological strategies—including SGLT2 inhibitors, GLP-1 receptor agonists, non-steroidal mineralocorticoid receptor antagonists, and lipid-lowering therapies—alongside structured lifestyle interventions. Ultimately, an integrated, multidisciplinary healthcare approach is vital to significantly reduce cardiovascular and renal complications while improving long-term clinical outcomes.
